Student Athletes - Health and Safety Information

Athlete Health and Safety Resources

Sports Physical
Effective Date: August 1st, 2022
Applicable to all campuses for students 5th grade and up
  • Athletic Participation Forms must be submitted annually before a student participates in any school try-out, practice, athletic class, open gym, athletic competition or travels with an athletic team for any purpose.

  • For the health and well-being of students, Aristoi Classical is requiring that all athletes grades 5th-12th participating in organized school-sponsored sports have a new Preparticipation Physical Examination (PPE), also known as a “sports physical”, given prior to the start of each school calendar year.

  • A Pre Participation Examination (PPE) differs from a “well-child” exam in that the focus is to “clear” the participant for specific activity. A PPE can help find health problems that could stop your child from competing safely.

  • Parents should print a physical and medical history form to take to your physician or school sponsored PPE event. The required form provided by UIL is available HERE.

  • To streamline the process, parents are encouraged to fill out the Medical History portion of the form prior to arriving at the students’ appointment or school sponsored PPE event.

  • Students are required to use the Pre-participation - Physical Examination Form provided. NO OTHER Physical Examination Form can be accepted.

  • Parents and Students are encouraged to watch the following Cardiovascular (Heart) Related Educational Videos prior to filling out the Medical History portion of the PPE and before arriving at the students’ PPE appointment.

  • Aristoi Booster Club will sponsor on-site PPE/Sports Physicals provided by Memorial Hermann Sports Medicine and UT Physicians in the Griffin Center on Tuesday July 26th from 6:00 PM- 7:00 PM for $25 each. Look for communication from Aristoi and ABC with additional details and instructions on how to RSVP.

  • If you choose not to participate in the ABC sponsored PPE sports physicals on July 26th, you can obtain one from your students’ pediatrician, local urgent care facility, or participating community clinic. Student athletes will still be required to use the PPE form provided.

  • Please refer to the The Texas Charter School Academic and Athletic League (TCSAAL) Handbook and COVID-19 Protocols for additional safety information.

  • For questions or concerns, please contact the Aristoi Classical Academy Athletic Department.
